The DDA has published an SOP on adverse event management.
This SOP describes good practice and good systems of care for reporting, learning, sharing, taking action and review of incidents involving medicines as part of a patient safety culture.
Harm or incidents arising from deliberate intended actions and those not involving medicines are excluded from the scope of this document.
Its purpose is to support dispensary teams to manage adverse events involving medicines in the dispensary and create a culture of patient safety.
