Alert: Omeprazole 20mg/15 ml oral solution/Tamoxifen 20mg film-coated tablets

Two class 3 recalls have been published:

One batch of Omeprazole 20 mg/15 ml Oral Solution

Affected Lot Batch Numbers

The recall is due to a faulty plunger stem and cup assembly, which could lead to a premature mixing of the two components of the medicinal product causing precipitation and discoloration of the product in the bottles.

Affected Lot Batch Numbers

The recall is due to dissolution failure during stability testing.

For both items, patients should continue to take medicines from these batches as prescribed by your healthcare professional.

Healthcare professionals are asked to quarantine all remaining stock and return it to your supplier using your supplier’s approved process.
