Dispensing intervals are facing scrutiny in Scotland.
In a new consultation on polypharmacy prescribing guidance, dispensaries are asked to review dispensing intervals (28/56 days) as well as use of costly formulations (eg, dispersible); branded products; costly unlicensed ‘specials’; multiple strengths or formulations of same medicines and medicines of low/limited clinical value.
Sustainable dispensing is one of seven steps in a proposed new approach for polypharmacy medication review in Scotland. Other steps cover medication reviews for those receiving care at home and in care homes, people at risk of falls, managing frailty and for specified conditions such as diabetes and hypertension.
The consultation aims to identify polypharmacy that puts people at greatest risk of harm and to agree a medication regimen that is tailored to their changing needs and expectations.
Dispensing is considered a key stage when appropriate polypharmacy requires consideration. Others include: prescribing (and risk assessment), medication review, communication and patient engagement, and medication reconciliation (at care transitions).
The consultation closes on September 22.
